OK, but what is your actual question? Lots of debug information about a hardware/driver related issue, but no question.

With a bit of duckduckgo-fu (using the error message you posted as a search term) I found this, which looks like exactly the same problem you are experiencing:

Several explanations there and possible causes.
From a brief read of the page, you should be using at least V1.4 of the firmware. And according to your post - you are using V1.4, so it's probably not the firmware.
The page also mentions that you should have kernel version 3.18 or greater (3.18 has some kind of patch for this issue). So if you are using anything less than V3.18 of the kernel - that is probably your problem. In which case - try upgrading your kernel to a newer version.
However, If your kernel version IS already 3.18 or greater - then it could be some kind of physical problem with the hardware itself, or it could be a problem with the USB controller attempting to start USB 1.1 in an unsupported mode.
