ufdbguard (aka urlfilterdb) and squidGuard are the first ones that come to mind. Another one is RedWood. These are proxy based web content filters, usually used on servers/gateways/firewalls. There are some Server and Firewall oriented GNU/Linux and BSD distributions implementing those filtering solutions.
