I re-read the thread and I also don't think you were offensive... she even liked your response to help her. I'm glad that she reported back what the resolution to the problem was for her, but it is not uncommon for folks not to return for long periods, if at all, after they fix the issue at hand. It is nice of you to apologize since you think it might have been an issue... that's all any of us can do.
